We are your advocates and will coordinate and explain your care to you in terms that are easily understandable. We are the direct line of communication between you and the healthcare team that treats you. As Registered Nurses, we will review your medical records and determine all of the treatment choices that you have and position you to make a truly "informed decision". We will create a comprehensive care plan with you and conference with you frequently so that we will be aware of any changes that you desire. We will implement and continually evaluate the plan to ensure that desired outcomes are being achieved. We will visit you in the hospital or facility and work with their professional staff to ensure that you are efficiently and safely discharged to the place of your choice. We will then assist you with community reintegration and home based services. The benefits of Medical Case Management are reduced length of stay for each patient, decreased re-admission rates, and a decline in wasteful utilization of resources.
